SALUD Smiles

SALUD has a new website offering you the opportunity to enroll in the SMILES program for any children in the Morgan County School District Re-3. SALUD's new electronic enrollment process allows families to enroll their child/children for dental services by visiting SALUD's website at www.saludschools.org. The electronic consent does not replace the paper consent forms completely, it just allows families another option for accessing our services. 

After completing the below form, choose the "Submit to Salud Clinic" button in the lower right corner, and the form will automatically be emailed to the Salud Clinic in Fort Morgan.

Después de completar el formulario, elija el botón "Enviar a Salud Clínica" en la esquina inferior derecha, y el formulario se enviará automáticamente por correo electrónico a  Salud Clínica en Fort Morgan. 

Please note that we have a full service medical clinic on the high school campus. This clinic is available to serve any of your student's medical needs. If you would like your student to be able to access these services, please complete the online registration at saludschools.org.  For a student to receive care without a parent present a parental consent form MUST BE SIGNED prior to the appointment.
